Holiday During Term Time
Headteachers are only able to grant a leave of absence during term time in exceptional circumstances. If you feel this applies, please read West Berkshire's Holidays in Term Time - A Guide for Parents before submitting a request to the relevant school(s) using this form. You are advised not to make any booking before ascertaining if the proposed holiday has been authorised.
Home School Agreement
Our re-designed Home School Agreement clearly sets out what we all need to do to create a secure, happy and caring school communuity. Pupils, parents and teachers all sign up reinforcing to children that we work together in partnership. If you haven't already done so, please sign the agreement and return it to the school office.
Medication in School
If your child needs medication administered during school hours, please complete this form and return to the school office. All medication must be in its original packaging with the child's name on it.
Breakfast and After School Club
If you would like to register your child for breakfast or after school club, please complete this form and return to the school office
Special Diet Form
If your child has an allergy and you wish them to have school meals, please complete and return this form to the school office.
